Terms for the international Accident Report Form service
These terms apply to the international Accident Report Form service operated by Schadebureau Nederland B.V., registration number 99746115. They are effective from 2026-10-01.
What these terms cover
These terms govern use of the English Accident Report Form website, the front and reverse-side form flows, participant invitations, signatures and PDF delivery. A report is the record created from information supplied by its participants.
Each participant accepts the documents presented for their part of the flow. The accepted document text, offer and time are recorded with that report so that later wording or pricing cannot silently change the agreement already made.
The service
Accident Report Form is a technical tool for entering, synchronising, reviewing, signing and downloading an accident report. It can support two participants with separate access and can create front-side and reverse-side PDFs from the information they submit.
The service is not an insurer, police or emergency service, law firm, medical provider, recovery service or claim representative. It does not decide fault, negotiate loss, notify every insurer automatically, guarantee legal validity or guarantee that an insurer or authority will accept the document.
Your responsibilities
Use only access intended for you, provide information honestly and as completely as reasonably possible, review shared facts and your own details before signing, respect other people’s privacy and download the completed PDF before its deadline.
You must not impersonate another person, upload unlawful or malicious material, interfere with security or availability, scrape the service, attempt to access another report, or use a personal link after learning that it was disclosed without authority. You are responsible for a device and email or telephone account used to receive access.
Other people’s information and signatures
Only enter another person’s information when it is relevant to the accident record and you have a lawful reason to do so. Tell the other person about the processing where the law requires it, and do not include unnecessary medical, financial or unrelated personal information.
Each participant is responsible for checking the content attributed to them. A signature records approval of the submitted report content at that time; it is not, by itself, a decision or automatic admission about liability, insurance coverage or the amount of any loss.
Free launch offer
The English front side and reverse side, including digital completion, signatures and PDF download, are free under the launch offer. There is no checkout and no payment record is created for a free report.
A report keeps the offer captured when it was started. A future paid offer can apply only to a newly started report under separately published terms and an implemented payment flow; it cannot reprice or remove access from an existing free report.
Temporary access and keeping your copy
A new front-side draft is initially available for two hours from its original start. If an authorised participant validly continues through the driver step, it may remain available for up to seven days from that original start. A linked reverse-side draft is bounded by the applicable front-side deadline, while a separately started reverse-side draft is available for up to three days.
A completed front-side or reverse-side report is available through the service for three days from the relevant first completion. Reopening, downloading, resending or switching language does not extend a deadline. The deadline shown with the personal access is controlling. Download and securely keep your own PDF in time; recovery after expiry is not promised.
Safety, reporting and insurer requirements
Safety and local legal duties come before using the service. Contact local emergency services or police when the situation or applicable law requires it, and follow any deadline or reporting instruction from the relevant insurer, employer, fleet owner or authority.
Downloading a PDF is not proof that an insurer or authority received it. Unless the service expressly confirms a specific delivery, you remain responsible for sending the document through the recipient’s required channel and keeping evidence of receipt.
Privacy and communications
Personal information is processed as described in the privacy notice. Personal links must be kept confidential. Necessary service messages may be sent to complete, secure or deliver a requested report; optional messages remain subject to the applicable choice and unsubscribe controls.
Intellectual property
The service, software, design and original site content are protected by intellectual-property rights belonging to the operator or its licensors. You may use and share the PDF created from your report for the accident, insurance, advice and legal purposes for which it was generated.
Except where mandatory law permits it, you may not reproduce, resell, reverse-engineer, interfere with or commercially exploit the service or its source materials without prior written permission.
Availability and responsibility
Schadebureau Nederland B.V. uses reasonable care to operate the service but does not promise uninterrupted or error-free availability. Keep your own copy and use another lawful reporting route if the service is unavailable or a deadline is approaching.
To the extent permitted by mandatory law, Schadebureau Nederland B.V. is not responsible for loss caused by inaccurate or unlawful user input, disclosure of a personal link by a user, a missed external reporting duty, an external service failure, or an insurer’s, authority’s or other third party’s decision about the report. Nothing excludes liability that cannot lawfully be excluded.
These terms are governed by the laws of the Netherlands. This choice does not remove mandatory consumer protections or access to a competent court that the law of a user’s country of residence provides.
Suspension, changes and ending use
Access may be restricted where reasonably necessary for security, suspected misuse, maintenance, law or protection of another person. The operator may improve or discontinue features, but will not silently convert an existing free report into a paid report or extend a stated retention period merely because the service changes.
You may stop using the service at any time. Stopping does not itself erase information that must temporarily remain for another participant, security, compliance or legal rights; the privacy notice explains retention and available requests.
